What the consult form collects

Your first name, mobile number, optional email, the area you want to discuss, virtual or in-person preference and rough timing. It does not ask for health history, medications, photos or any medical detail. Please do not include health details in the form or the chat.

Health information

Health history is taken at the consult through the practice's secure patient system. Any vendor that handles protected health information signs a business associate agreement with the practice.

The front desk chat

The chat answers general questions and offers times. It is not medical advice, and a person can take over at any point. Chat text is kept only as long as needed to reply and book, then deleted.

Cookies and advertising

This site uses no advertising pixels and no tracking on form pages. Basic, privacy-friendly visit counts may be used to see which pages help people.

Text messages

If you ask for times by text, you receive messages about your consult only. Reply STOP at any time.
